Each year the OPHA honors persons and programs that have made special, noteworthy contributions to the field of public health in Oklahoma.
The Association invites each of you to nominate individuals and programs that you feel are especially
worthy of such recognition. Please write a brief narrative, not to exceed 2 pages, (font size 12) and describe the reasons for your nomination, using the categories and criteria schedule listed herein. You may send support information of no more than two pages and two supporters’ letters (one page per letter). All correspondence (maximum of 6 pages) must be typewritten. The deadline is February 1, 2012.
OPHA members and the communities in which they serve are composed of many committed, dedicated and talented candidates. Please give this matter some thought. We look forward to hearing from you.
The Awards presentations will be held at the annual OPHA Conference (date and location pending)
Presentations will be made at during the Conference.

Contributed to implementation or expansion of new or existing services
Does not receive income from any field of health care
Does not have to be a member of OPHA
Contribution to the Field of Public Health
Criteria:
Career in public health
Major impact on public health
Long-term significant contribution to the field
Does not have to be a member of OPHA
Special Business Award for Excellence in Public Health
Criteria:
Made a significant achievement in public health activities and issues
