Scholarships

Scholarship Application       

The Oklahoma Public Health Association (OPHA) is committed to enhancing educational opportunities in public health.  OPHA is taking applications for its annual academic scholarship to be awarded to an OPHA member(s), enrolled full or part-time, post high school.  Applicants must be enrolled in a school that is sponsored by or accredited by an Oklahoma based college or university.  The amount to be awarded this year will be up to a total of $1000.00 to be distributed among one or more students.

Student membership is $10.00.  Others, who may apply for the scholarship but may not qualify for student membership, or are not covered under a corporate membership pay dues of $30.00 for a one-year membership. You do not have to be a member to apply for the scholarship.

The scholarship application that contains eligibility criteria and application requirements.  Completed applications and necessary documentation must be postmarked by March 15, 2013 and mailed to:

The selection will be made and all applicants will be notified by March 29, 2013.  The recipient also will be announced at the Association’s annual conference at the Reed Center, 5800 Will Rogers Rd., Midwest City, OK, on April 4, 2013 at the Awards Ceremony.
